Here are the forms for medication for the Washington, D.C. trip. ALL medication needs a separate form with a doctor's signature.
- Self-Administration of Medication Form (if you want the student to carry his/her own medication
- Medication Form for EACH medication
You only need to fill out an Action Plan if this pertains to your child.
